 What does an Indiana apostille look like?

In the State of Indiana, as in all states of the United States of America, an apostille is a separate page attached to the document. It has the  Seal of the State of Indiana (black and white image) and is stapled to the document it authenticates. On top of a staple, in the upper left corner, there is a foil gold seal glued to the document. As required by the Hague Apostille convention, each apostille has the title "Apostille", the reference to the Hague Convention, and the following ten items: (1) country where the apostille is being issued ("United States of America"); (2) name of the official (whose signature is being certified by an apostille, in the image above it is "Luis Galup"; (3) the position or capasity of this official ("St. Joseph County Health Department"); (4) name of of the seal ("Indiana Secretary of State", (5) name of the city where the apostille is being issued ("Indianapolis, Indiana"); (6) the date when the apostille is being issued; (7) position of the official person who signed the apostille ("Indiana Secretary of State"); (8) consecutive number of the apostille (in the image above it is "A2017-0519104326"; (9) the seal of the apostille issuing agency (Seal of the State of Indiana); (10) signature (the signature is facsimile, black in color).

Indiana apostilles also have additional language at the bottom. "Effective May 1, 2003 all apostilles from Indiana Secretary of State have an electronically printed seal. This document was revised March 17, 2012"
